Father/mother consent for application of child passport in South Africa:

1. The parent in South Africa should approach the Office Manager of the Home Affairs office where the application will be lodged
2. An email address of an official at the Department of Home Affairs who will accept the application
3. Complete form DHA-73
4. A certified copy of the passport of the father/mother (who is living in Canada)
5. A certified copy of the child’s passport
6. A certified copy of the child’s unabridged birth certificate
7. A certified copy of the passport of the father/mother in South Africa
8. Once you have the above please send all certified documents to the following address:
